0215(3): anti-fungal medication in waddler classroom did not have a medication authorization form available during visit; instructions require physican approval based on the instructions as the child was under 2 years of age. 0215(3)(a)(iii: multiple tubes of diaper ointment were observed labeled with only the child's first name only.
